Below are some of the common forms required for leagues, clinics, and other activities at TSE.  Click on the form name at left to download. Please call if you have any questions.

Registration Forms

• General League Registration - Generic form that can be used for any league registration
• General Tournament Registration - Generic form that can be used for any tournament registration
• Camp & Clinic Registration Form - Generic form that can be used for any camp, clinic, or academy registration

Waiver Forms

• Team Roster/Waiver Form - Both a roster and waiver form for the team.  Everyone who plays must fill out this form (parent or guardian if under the age of 18)
• Individual Waiver Form - Individual release of liability form.  Signed by individual (parent or guardian if under 18), can be used for all activities.
