Players Membership / Waiver
We have created a Player's Membership to track our facility waivers. The free membership is needed for both adults and youth and is good for 1-year from completion.
Once membership is completed and a picture is added in our system, participants can check in with the membership as needed. If membership is not complete, proof of ID and a new waiver is required for each visit.
The Membership can be completed two different ways:
- Online: Click the button below to “purchase” the free membership. An account is needed to create the membership.
- Hard Copy: Click this link to download a hard copy form to submit in person ( Players Membership / Waiver).
A parent or guardian can complete the membership for themselves and their child(ren).

- General League Registration – Generic form that can be used for any league registration
- General Tournament Registration – Generic form that can be used for any tournament registration
- Camp & Clinic Registration Form – Generic form that can be used for any camp, clinic, or academy registration
Miscellaneous
- TSE Responsibility Code – Official facility policies
- Team Roster – A coach/manager must complete this form for each season of competition
- Code of Conduct – All coaches/managers must complete this form for each season of competition
- TSE Concussion Protocol – Our concussion protocol procedures and recommendations
